Minor Work Permits

Work permits are needed for minors under the age of 18 who are employed during the school year. Additionally, 14 and 15 year olds need a work permit for employment at any time, including summer break. A new work permit will need to be completed for each company a minor works for. Please see the following link from the Department of Commerce regarding minor work permits.

To obtain a work permit, please complete the following:

• The Parent Guardian Consent Form (completed by parent or guardian)
• The Student Applicant Information (completed by Parent/Guardian) / Pledge of Employer (completed by the employer)
• The Physician's Certificate (completed by a medical doctor)

Once pages are completed, bring the forms to the Gallipolis City Schools Administrative Office located at 61 State Street, Gallipolis, Ohio 45631 and the information will be submitted to The State of Ohio to generate the work permit. If you have any questions please feel free to call 740-446-3211.

•  Pre-Application Form -  Proof of birth date is not needed if the student's birth certificate is uploaded on Final Forms to access.
• Physician's Certicate - Physicians' Certificate is not needed if a current sports physical is uploaded on Final Forms to access.
