Student Registration

 

Directions:  Click the link, choose the blue button under Parent to set up a new parent account. (If you already have an account with Final Forms, please see the alternative directions.) →

Enter your parent information and email. Final Forms will then send you an email to confirm your account. Then you can start to enter your students' information. Once it is complete, please don't forget to upload documents to complete the registration process.

The following items are required to complete the registration process:

• student's birth certificate 
• social security card 
• immunization record 
• Parent proof of residency 
• photo ID uploaded for the parent 
• If there are any court documents for custody

Alternative Directions: If you already have an account with Final Forms, please login to your parent account using the link above. Once you are logged in, you will be able to update your student or add them to your account whichever fits your situation. Please don't forget to upload documents listed above to complete the registration process. 

***Important enrollment reminder***

*Registration is not complete until ALL parent forms are completed on Final Forms and ALL documents are uploaded.

*Enrollments received through Final Forms after 12 pm will be processed the next business day. Please do not send your student to school without a confirmation email from Final Forms/Gallipolis City Schools.
